Seeking an experienced Registered Nurse (RN) to serve as a Health Care Surveyor and Administrator for Licensed Home Care Services Agencies (LHCSAs) across New York State. This position includes conducting regulatory surveys, managing complaint investigations, and ensuring agency operations align with state healthcare standards.
🔹 Responsibilities:
Conduct on-site re-licensure surveys, focused surveys, and complaint investigations for LHCSAs
Perform home visits, staff interviews, and documentation reviews (e.g., patient records, QA minutes, policy manuals)
Assess agency compliance with Article 36 and Department of Health regulations
Prepare and submit Statements of Deficiencies (SODs) using the ASPEN software system
Oversee administrative compliance and daily operations of LHCSAs
Coordinate travel and prepare materials for field inspections
Maintain accurate documentation and participate in team debriefs
Collaborate on special projects and support ongoing quality improvement efforts
Be available for any legal proceedings related to enforcement actions
🧾 Qualifications:
Active Registered Nurse (RN) license in New York State
Associate or Bachelor’s degree in Nursing
Minimum 2 years of clinical experience (home care preferred)
Working knowledge of NYS home care regulations and Article 36
Strong critical thinking, communication, and organizational skills
Proficient in Microsoft Word and Excel
Ability to travel regionally for site visits
Professional demeanor and commitment to one-year term, pending satisfactory evaluation
