Are you a licensed social worker with a passion for quality care and compliance? We’re looking for a dedicated LHCSA Surveyor to support policy review, licensing surveys, and operational assessments for Licensed Home Care Services Agencies (LHCSAs) across New York State.
What You’ll Do:
Conduct pre-opening and re-licensure surveys for LHCSAs
Review policy & procedure manuals for compliance with Article 36 regulations
Perform on-site visits, assess documentation, staff readiness, and agency procedures
Draft compliance reports and Statements of Deficiencies (SODs)
Collaborate with internal teams and regulatory bodies to ensure readiness and adherence to standards
What You’ll Bring:
Master’s in Social Work and active LMSW or LCSW license (NY)
2+ years of experience in social work (home care preferred)
Strong attention to detail and critical thinking skills
Excellent communication and writing abilities
Proficiency in Microsoft Word & Excel
Flexibility to travel for field work
